What Aetna typically covers for SUD

Most Aetna NJ commercial plans cover the full SUD treatment continuum required by the federal Mental Health Parity and Addiction Equity Act (MHPAEA): outpatient, IOP, PHP, residential, detox, and MAT. Specific benefits vary by plan — self-insured employer plans can have different benefit structures than fully-insured plans. Prior authorization for Aetna SUD benefits

Most Aetna plans require prior authorization for levels of care above outpatient. The facility typically handles this on your behalf, but the clinical criteria are Aetna’s (not the facility’s). Expect criteria to cover medical necessity, clinical appropriateness of the level of care, and in some cases an ASAM-criteria assessment. Denial is not automatic — prior auth is a review, not a gate.
